Policies Acceptance Agreement

Financial Policies

• A yearly non-refundable registration fee of $35 is required per student.  This fee covers any and all classes taken in the 2010-2011 school year, except Hip Hop Dance.  This class require their own registration with the specific provider. 

• No refunds or make ups will be given for student absences for group classes.

• Lessons canceled due to teacher absences will be rescheduled. No refunds will be given.

• Students will be automatically re-enrolled each month.  Please notify Carol Green if your student will not be re-enrolled for the next month

• Yearly prepaid tuition will be refunded on a prorated basis.

• A late pick up fee of $20 per instance will be charged for parents who are repeatedly late.

• A returned check fee of $20 will be assessed.

• The school requires 30 days written notification of student withdrawal (or one month's tuition).

General Policies

• I agree for my child( or myself) to participate in  Sugar Hill School of the Arts activities including but not limited to karate, music lessons, art, cooking, tap, ballet or other activities that may be hazardous or otherwise involve a risk of physical injury or death to participants (the “Activities”).

•  I expressly assume any and all risks of injury or death arising from or relating to the Activities and waive and release any and all actions, claims, suits or demands of any kind or nature whatsoever against Sugar Hill United Methodist Church or it’s corporate affiliates, contractors, vendors, officers, agents, sponsors, volunteers or representatives of any kind (collectively “Releasees”) arising from or relating in any way to my child’s (my) voluntary participation in the Activities. I understand that this SHSA school polices acceptance agreement means, among other things, that if my child (myself) is injured or dies as a result of his/her participation in any of the Activities, I and/or my family or heirs cannot under any circumstances sue Releasees or any of them for damages relating to or caused by my child’s (my) injuries or death.

•  I agree to indemnify Releasees and their subrogates, if any, in the event of any loss, damage or claim arising from or relating in any way to my child’s (my) participation in any of the Activities.

• In event of an emergency, I hereby authorize SHSA administration as an agent for me, to consent to: any x-ray examination; medical, dental or surgical diagnosis; treatments; or hospital care advised and supervised by a physician, surgeon or dentist (as appropriate) licensed to practice under the laws of the state where services are rendered, either at a doctor’s office or in a hospital. I expect the student’s family will be contacted as soon as possible.

• I hereby give SHSA permission to reproduce my child’s (my)likeness in church or SHSA literature, newspaper articles, inserts, or videos.

• I understand that if my child is disruptive during class, the teacher will use time out as a form of discipline.

I have read and understand and agree to the policies and procedures stated in the SHSA School Policies Acceptance Agreement.
